Name *

This person has a potentially life-threatening allergy (anaphylaxis) to:

Epinephrine Auto-Injector:

Dosage:

A person having an anaphylactic reaction might have ANY of these signs and symptoms:

•  Skin system: hives, swelling (face, lips, tongue), itching, warmth, redness

•  Respiratory system (breathing): coughing, wheezing, shortness of breath, chest pain or tightness, throat tightness, hoarse voice, nasal congestion or hay fever-like symptoms (runny, itchy nose and watery eyes, sneezing), trouble swallowing

•  Gastrointestinal system (stomach): nausea, pain or cramps, vomiting, diarrhea

•  Cardiovascular system (heart): paler than normal skin colour/blue colour, weak pulse, passing out, dizziness or lightheadedness, shock

•  Other: anxiety, sense of doom (the feeling that something bad is about to happen), headache, uterine cramps, metallic taste

Early recognition of symptoms and immediate treatment could save a person's life.

Act quickly. The first signs of a reaction can be mild, but symptoms can get worse very quickly.

1. Give epinephrine auto-injector (e.g. EpiPen®, ALLERJECT®, EmeradeTM) at the first sign of a known or suspected anaphylactic reaction. (See attached instructions.)

2. Call 9-1-1 or local emergency medical services. Tell them someone is having a life-threatening allergic reaction.

3. Give a second dose of epinephrine as early as 5 minutes after the first dose if there is no improvement in symptoms.

4. Go to the nearest hospital immediately (ideally by ambulance), even if symptoms are mild or have stopped. The reaction could worsen or come back, even after proper treatment. Stay in the hospital for an appropriate period of observation as decided by the emergency department physician (generally about 4-6 hours).

5. Call emergency contact person (e.g. parent, guardian).

(Please read and understand carefully)

• By providing your child’s health information, you (parents/guardians) consent to its collection, use, and disclosure strictly for school-related purposes. This information will only be accessible to authorized school personnel who have a legitimate need to know for your child’s care and safety.

• Parents/guardians acknowledge the rights granted under the Alberta Health Information Act, including the right to access their own health information, request corrections to inaccuracies, or add relevant information when necessary.

• I understand that monthly school fees will be debited from my account on the 1st of each month for the entire school year (October 2025 to June 2026) and agree to abide by this year’s contract. If unforeseen circumstances require my child to transfer to another school, I agree to provide a written one-month notice by the 1st of the month. The school will decide whether to honor this request on a case-by-case basis. Notices given after the 1st of the month will result in the full fee being charged for the next calendar month. Any outstanding fees or balances must be settled before records and documents are released.

• I acknowledge that it is my responsibility to inform the school of any changes to our address, phone numbers (home or work), emergency contacts, or other significant matters affecting my child’s well-being (e.g., illness, death in the family, divorce, separation, etc.).

• I will notify the school daily via the school app if my child is sick and will keep my child at home if they show symptoms of vomiting, fever, diarrhea, excessive coughing, or other contagious conditions such as "pink eye."

• I will inform the school in advance if my child is expected to be absent for more than three consecutive days, except during scheduled winter or spring breaks. I understand that there will be no refunds for absences or extended leaves (e.g., vacations).

• I grant permission for Calgary International Academy staff to provide basic first aid for minor injuries and to seek appropriate medical support (e.g., ambulance, poison control) in the case of serious injuries requiring immediate care or hospitalization.

• A $50 CAD fee will be charged for each NSF (Non-Sufficient Funds) transaction. Persistent payment defaults may result in the cancellation of services.

.
I have read, understood, and agree to the terms of this contract, including the Fees Policy for school year 2026-27 and I have read and understood the Parent’s Handbook including Policies and Procedures.
I authorize the school to use my child’s pictures, videos or any form of media for promotional purposes. *
